This notice describes the information collected by this golf tournament registration form.

What the form collects

Team name (or your name if playing as a single), package selection, primary contact name, email and phone number, golfer names, polo or hat choices and polo sizes, any tournament notes you choose to provide, and whether you agree to share your name and email with sponsors. Teammates who join with a team invite link enter their own name, polo or hat choice and polo size, and may add a phone number and email address. Do not include medical records or other sensitive information in the notes.

Team links

Each registration gets a private team page link for the captain, and teams get an invite link to share with teammates. The invite page shows the team name and each golfer's first name and last initial. The team page shows full golfer names and lets anyone with the link edit the team, so captains should keep it private. Teammates' phone numbers and email addresses are visible only to EVS, not on either page. EVS can replace either link at any time.

How it is used

EVS uses the information to organize teams, prepare apparel, manage tournament registration, and communicate with the primary contact. Relevant roster information may be shared with Ridge Creek to prepare for the tournament. The form does not collect card or bank-account information.

Sharing with sponsors

The registration form has an optional box that lets EVS share your name and email address with sponsors of this tournament, so they can contact you about their businesses. It is unchecked unless you check it, and leaving it unchecked does not affect your registration.

If you check it, EVS shares only your name and email address. Phone numbers, apparel details, payment status, and notes are never shared with sponsors. Each box covers only the person who checks it: a captain cannot agree for teammates, but teammates who join with the invite link can choose for themselves.

Once a sponsor receives your information, their own privacy practices apply. To stop future sharing, call EVS at (559) 648-7300. EVS cannot recall information that was already provided to a sponsor.

Where it is kept

Registrations are stored in the EVS Events WordPress database. The registration management area and roster exports are restricted to site administrators. Roster downloads contain personal information and should only be shared with people who need it for tournament operations.

Payments

Player and sponsorship payment links take you to Elite Valley Sports' Square checkout. Information you submit there is handled by Square and its payment services.
